Petra Player Registration

I am a parent or legal guardian of the named minor. I fully understand that participating in the sport of soccer presents a risk for serious injuries. I have made a conscious decision to allow the named minor to play and confirm my son’s/ daughter’s participation for the Petra Sports Academy's training programme .

 

I hereby authorize the staff of Petra Sports Academy, a division of PSA- Petra Sports Academy (OPC) Private Limited to act to their best judgement in any emergency requiring medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are deemed necessary to preserve the life, limb, or well-being of the registrant.

 

I hereby waiver and release Petra Sports Academy and its staff from any and all liability for any injuries incurred while playing football, training or traveling to or from the academy. All medical expenses in such a case will be the parents responsibility.

I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities.

 

Petra Sports Academy is not responsible for items that are lost, stolen or damaged. I acknowledge and agree to assume and be fully responsible for all property or other damage to the facilities used at the academy. Misbehavior or indiscipline and the participant will be asked to leave the program.

 

I understand Petra Sports Academy retains the right to use any photographs, videotapes, motion picture recording or any other record of event publicity, advertising for any legitimate purpose.

 

I shall observe and comply with all rules, regulations and guidelines of Petra Sports Academy.
